DWP Onboarding Form Instructions
1. Form Signer Information
• LLC or Retirement Account → Only one signer is required.
• Trust Accounts → Multiple signers may be necessary, including trustees. Consult with your estate professional.
2. Account Types
• If your estate is complex or involves multiple entities, consult your attorney to confirm which entity should hold the assets before completing this section.
• If your IRA or ROTH was inherited, please inform a DWP advisor.
3. Primary Account Holder Information
• This section is for the individual account holder’s personal details.
• Ensure all personal information is accurate and matches legal documentation.
4. Secondary Account Holder (If Applicable)
• If there is a secondary account holder, they will complete their section separately via their own DocuSign process.
• This section will automatically populate once the secondary account holder submits their information.
5. Entity as an Account Holder (If Applicable)
• Only if an entity (such as an LLC or trust) is a secondary account owner
